Purpose of the Program
The Well Child Health Assessment Program assesses children for normal development and growth. Children 0-18 years of age are given an examination that may include:
- Complete physical examination
- Developmental screen
- Education and information pertinent to your child's age and assessment findings
- Fluoride Varnish
- Growth assessment (height and weight)
- Gynecological exam deferred and done as needed through the Family Planning / Well Woman Programs
- Hearing screening
- Lab Testing as indicated
- Immunization status review and immunizations administered as needed
- Lead screening
- Referral to health care provider and other resources as indicated by exam
- Vision screening

Fees
There is a sliding fee scale available based your household gross income and your household size.
Enrollment / Appointment Process
Call 785-457-3719 for appointment. Complete the papers that are mailed to you. Come with your child to the appointment that will last about 1.5 hours.
