| Well Child Health Assessment Program |
Purpose:
To assess children for normal development and growth. |
Description:
Children age 0-18 years of age are given an examination that includes:
Complete physical examination (gyn. exam deferred and done as needed through the Family Planning/Well Woman Programs.
Developmental screen.
Growth assessment (Height and weight).
Vision screening.
Hearing screening.
Immunization status review and immunizations as needed.
Fluoride Varnish.
Lead screening
Hemoglobin testing (finger stick).
Urine screen.
Education & information pertinent to your child's age and assessment findings.
Referral to health care provider, local resources including the Infant and Toddler Program, and to the
county Dental Plan as needed and as eligible. |
Eligibility:
No well Child Coverage, Medicaid/Healthwave, No Health Insurance. |
Fees:
Sliding fee scale available, we will need to know your household income (gross) and your household size

Enrollment:
If your child has a medical card and is signed up with a doctor under the Health Connect Program, you
will need to get a referral from that doctor to be seen at the Health Department.
Call for appointment at 457-3302, receptionist will assist you with an appointment time.
Complete the papers that are mailed to you.
Come with your child to the appointment that will last about 11/2 hour.
Plan to pay for visit with cash, check, or Medical card. |
