Purpose:
To assess an individual's risk for chronic disease.
To reduce those risks related to tobacco use, poor dietary habits, and lack of regular physical activity
through individual and class education and information.
Fiscal agent and member of Pottawatomie County Wellness Coalition. |
Description of program:
Participants complete a survey, provide a blood specimen for cholesterol testing after fasting
for 8 hours, and receive blood pressure testing and weight and height measurement.
After laboratory test results are obtained, participants visit the Health Department for test results,
counseling, and education.
Referrals are made to health care providers or dieticians as needed. |
Procedure for accessing the program:
1) Call for appointment 785-457-3302.
2) Need Doctors Order
3) Fast for 8 hours and have blood drawn at participating laboratory, Westmoreland Clinic. |
Fees:
Sliding fee scale available as desired as based on income and family size. |
